surveys expired?

Hello! We are having a problem with the surveys. When our patients click to send it, it gives them a screen that says that they are expired. One patient in particular I know opened it when it was recieved. I am pretty sure that this is happening alot because we havn't received on in a while. Is this a setting we have wrong? Please help.

Yes, it is happening because you have not selected any questions for the survey. The blank survey was sent to 167 patients. Please add at least one question or use our templated survey. Thank you.
